Qualifications

• Registered Nurse (RN) required, in good standing.
• Minimum 2 years of related nursing experience and 1–3 years in a supervisory capacity.
• Knowledge of state and federal laws for skilled nursing facility operation (Colorado).

Responsibilities

• Coordinate the work of team members to meet resident goals and expectations.
• Lead post-admission screening and PAM-based activation coaching.
• Oversee discharge planning, care conferences and rehab/transition goals.
• Coach residents and families to improve plan of care and self-management.
• Support quality improvement practices and QAPI initiatives.
