Job summary

One Community Health seeks a Registered Nurse (RN) Care Team Case Manager in Stevenson, Washington. The RN CTCM will provide care coordination, case management, and acute interventions, supporting a clinical team in a full-time capacity. Requirements include a current Washington nursing license, experience with primary care, and excellent communication skills. Qualifications

• Graduate of an accredited nursing program.
• Registered Nurse with current Washington license and ability to obtain within 3 months of start date.
• Experience with adult and pediatric primary care.
• Current CPR/Basic Life Support (BLS) certification.

Responsibilities

• Provide care coordination and case management.
• Manage patients through transitions of care.
• Perform acute interventions following established protocols.

Skills

Strong written and oral communication skills

High level computer skills

Knowledge of customer service and workplace safety

Able to prioritize and organize work

Able to perform medical equipment and IV skills

Education

Graduate of an accredited nursing program
