Job summary

A Community Based Care organization seeks a dedicated RN Case Manager to oversee and coordinate comprehensive resident care in Portland, Oregon. The role requires a Registered Nurse (RN) license in Oregon and offers competitive compensation starting at $41.82 per hour. Responsibilities include managing nursing care, conducting assessments, and acting as a liaison among residents, families, and healthcare providers. Ideal candidates will have experience in behavioral health and strong communication skills.

Qualifications

• Knowledge of Oregon Community based Care regulatory compliance and healthcare laws preferred.
• Ability to analyze complex situations, identify solutions and make informed decisions.
• Demonstrated ability in working effectively with other staff and working independently with little supervision.

Responsibilities

• Lead the care coordination for the assigned floor.
• Complete comprehensive assessments on new residents.
• Collaborate with state Case Managers and medical professionals.

Skills

Excellent communication and interpersonal skills

Ability to assess patients’ needs

Experience working with individuals who demonstrate challenging behaviors

Proficiency in electronic health records

Education

BSN preferred

Registered Nurse (RN) licensure in Oregon required
