Job summary

ComForCare seeks a Registered Nurse to perform non-clinical in-home assessments, focusing on conversation, observation, and care planning rather than direct medical tasks. This role involves traveling to clients’ homes to gather health histories and develop practical care plans for in-home caregivers to implement.

Qualifications

• Active RN license in good standing.

Responsibilities

• Travel to clients' homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
