Columbia County Health System in Dayton, WA is seeking a dedicated Registered Nurse Care Navigator to join the Clinic Services team. This full-time, on-site role focuses on outreach, education, care coordination, and ongoing support for high-risk patients with complex medication regimens and missed appointments.

You will develop individualized care plans, coordinate across providers and community resources, document progress, and contribute to CQI efforts within a HRSA-funded program.

Qualifikationen

• Current Washington State RN license in good standing.
• Experience in patient education, care coordination, case management, or population health.
• Ability to work independently and with clinical teams.
• Strong patient communication, organization, and documentation skills; ability to learn EHR and tracking tools.

Aufgaben

• Identify and outreach to eligible high-risk patients.
• Assess clinical, care coordination, and social needs.
• Administer PAM and guide interventions.
• Develop individualized care plans with patients and care team.
• Coordinate care among PCPs, specialists, pharmacies, behavioral health, and other services.
• Coordinate medication reconciliation for polypharmacy patients.
• Connect patients to community resources via referrals.
• Document progress, barriers, and outcomes over time.
• Support patients reconnecting with primary care after missed appointments.
• Maintain documentation to support care-navigation billing.
• Participate in CQI activities and PDSA cycles.
• Collaborate with Director, Data Analyst, and partners to evaluate and improve the program.
