Job summary

Saint Luke's Primary Care Clinics is looking for a Regional Nurse Care Coordinator to support the Population Health team. This RN will coordinate care for a diverse patient population, focusing on education and outcomes. Monday–Friday, 8-hour days, no nights or weekends, with license requirements in MO & KS.

The role emphasizes collaboration with multidisciplinary teams and building relationships with patients, families and partners to drive population health goals.

Qualifications

• 3–5 years RN experience (or equivalent).
• Bachelor's Degree in Nursing required.
• MO & KS RN license required.

Responsibilities

• Engage with regional Saint Luke's Physician Group clinics to impact patient care.
• Drive engagement strategies for improving care practices based on guidelines and benchmarks.
• Collaborate with physicians, practice teams and plans to coordinate patient care and education.
