Job summary

Familyhealthcenter is looking for a part-time RN Care Coordinator in Mercer, WI. Qualifications

• Minimum of three years of recent clinical experience in a hospital or clinic setting working with adult patients.
• Demonstrated experience providing patient interaction and support by telephone.
• Bilingual in English and Spanish preferred.

Responsibilities

• Develops, implements, coordinates, and evaluates individualized healthcare plans.
• Conducts post-discharge follow-up calls for high-risk patients.
