Job summary

Provider Partners Health Plan is seeking a Nurse Care Coordinator to lead assessment, planning, and advocacy for PPHP members within the Model of Care. The role focuses on coordinating care for long-term care residents with complex conditions to achieve quality, cost-effective outcomes.

Active RN license in the practicing state with at least 3 years of clinical care experience is required. SNF/long-term care experience and Medicare/Medicaid knowledge are preferred, with strong teamwork and

Qualifications

• Active RN license in the practicing state.
• Minimum of 3 years clinical care experience.
• SNF/Long Term Care or case management experience preferred.
• Ability to work effectively with an interdisciplinary team.
• Knowledge of Medicare/Medicaid principles.

Responsibilities

• Identify risk and assess needs of PPHP members.
• Collaborate with Transitional Care Team to reduce readmissions.
• Document comprehensive assessment of barriers to care.
• Coordinate care plan development with nursing and interdisciplinary team.
• Facilitate referrals/consultations.
• Document care plans in the gEHRimed EMR.
• Evaluate outcomes and follow up on plan goals.
• Provide Model of Care training to staff and contractors.
