Job summary

Virginia Mason Franciscan Health is seeking an RN Care Coordinator to be a central figure in patient care, guiding the healthcare journey and coordinating transitions across inpatient and post-acute services.

You will assess, plan, and advocate for patients, collaborating with physicians, nursing, and providers to ensure timely, high-quality discharge planning and seamless care handoffs. RN license and WA state registration required.

Qualifications

• Graduate of an accredited nursing program with at least 2 years of acute hospital clinical experience.
• WA state Registered Nurse license still required.
• CPR certification within 90 days of hire.

Responsibilities

• Completes and documents discharge planning assessments and updates plan.
• Facilitates multidisciplinary discharge planning with patient, family, and post-acute providers.
• Oversees and evaluates the implementation of the discharge plan.
• Collaborates with the healthcare team to ensure progression of care and appropriate inpatient resource use.
• Coordinates transitions to the most appropriate post-acute services based on patient needs.
• Identifies readmission risk and implements interventions for at least 30 days.
