Qualifications

• Graduate of an accredited school of nursing.
• Minimum two (2) years of acute hospital clinical experience, upon hire.
• WA RN licensure, upon hire.
• Basic Life Support - CPR within 90 days.

Responsibilities

• Completes and documents discharge planning assessment for patients identified by screening process.
• Facilitates development of a multidisciplinary discharge plan with patient, family, and post-acute providers.
• Oversees and evaluates the implementation of the discharge plan.
