Job summary

Qualifications

• Graduate of an accredited school of Nursing.
• Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing in lieu of one year.
• Current RN license in Washington (RN-WA).
• BLS certification within 3 months if located in hospital.
• Understanding of utilization management and case management integration.
• Thrive in a fast-paced, self-directed environment.
• Knowledge of CMS standards and requirements.
• Highly organized with excellent time management.

Responsibilities

• Assess, plan, and facilitate comprehensive care across the continuum.
• Advocate for patients while coordinating with physicians, nursing, departments, insurers, and post-acute providers to ensure timely, high-quality transitions.
• Utilize clinical acumen and communication to optimize patient care across touchpoints.
