Qualifications

• Requires a nursing degree from an accredited school.
• WA nursing license (RN) required.
• BLS/CPR certification within 90 days of hire.
• Master's in Case Management or Nursing preferred, with experience.

Responsibilities

• Completes and documents discharge planning assessments and updates the plan as needed.
• Develops a multidisciplinary discharge plan with the patient, family, and post-acute providers.
• Oversees and evaluates the discharge plan implementation.
• Collaborates with the care team to optimize inpatient resource use per guidelines.
• Coordinates with post-acute services to ensure timely, appropriate transitions.
• Identify readmission risks and implement 30-day mitigation interventions.
