Job summary

Research Medical Center in Missouri is seeking a skilled Case Manager (RN) to support patients and families through their care journey. You will coordinate services, connect resources, and develop individualized care plans.

You will collaborate with physicians, nurses, and the Rehab Program Director to ensure safe transitions, clear communication, and high-quality, patient-centered care. This full-time role Monday–Friday day shift, with competitive benefits.

Qualifications

• 1 year experience required.

Responsibilities

• Guide patients and families through program orientation, benefits explanations, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to support participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs to the care team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
• Promote compassionate, respectful, and excellent care in line with organizational values.
