Job summary

SolutionHealth is seeking a Nurse Care Coordinator in Manchester, NH to organize team-based care planning and provide support for individuals with complex needs. You will partner with patients, families, and clinicians to develop holistic, timely plans across the care continuum.

The role emphasizes education, self-management coaching, and helping patients access community resources to achieve high-quality, patient-centered care.

Qualifications

• Graduate of an accredited nursing program required.
• Current nursing licensure in New Hampshire required; Massachusetts licensure in addition preferred.
• 5+ years nursing experience in acute, skilled nursing, community health or ambulatory settings.

Responsibilities

• Manages a panel of active patients requiring complex care management and care coordination support.
• Collaborates with the multidisciplinary team, including the patient and family, to design and implement holistic plans of care.
• Assesses patient and family health and social needs and health literacy; identifies strategies to resolve needs.
• Promotes patient engagement in self-care management and creates patient-directed care plans.
• Provides chronic disease self-management education and coordinates access to resources and support.
• Monitors adherence to care plans and facilitates changes as needed.
• Facilitates access to medical, specialty, and community providers.
