Job summary

SolutionHealth is hiring a Nurse Care Coordinator in New Hampshire to organize team-based care planning for high-risk, complex patients. You will work with patients, families, and a multidisciplinary team to develop holistic care plans and coordinate access to community resources.

You will educate patients on self-management, monitor progress, and adjust plans to achieve timely, cost-effective, high-quality care that is patient- and family-centered.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current NH nursing licensure required; MA licensure preferred.
• 5+ years nursing in acute, skilled nursing, community health or ambulatory settings; care coordination exp preferred.
• Experience with an ACO or value-based contracts preferred.

Responsibilities

• Coordinate care plans for patients with complex needs.
• Collaborate with multidisciplinary team and families to design holistic care plans.
• Assess health and social needs and health literacy; identify strategies to resolve needs.
• Educate patients and home caregivers on self-management and resources.
• Monitor adherence to care plans and adjust actions as needed.
• Facilitate access to medical and community providers and services.
• Support patients in level-of-care decisions over time.
