Responsibilities

• Manages a panel of patients requiring complex care management.
• Collaborates with the multidisciplinary team to design holistic care plans.
• Assess patients and families for unmet health and social needs and literacy.
• Promotes patient engagement and self-management through care plans and coaching.
• Provides chronic disease self-management education to patients and caregivers.
• Monitor adherence to care plans and adjust as needed.
• Facilitate access to community resources and financial assistance.
• Support access to appropriate medical and specialty providers.
