Job summary

Talented Medical Solutions seeks an RN Care Manager to coordinate patient care across the healthcare continuum in California, developing individualized care plans and partnering with patients, families, physicians, and teams to support positive outcomes.

You will manage referrals, coordinate services, monitor progress, assist with discharge planning, and educate patients about treatment options and available resources while ensuring regulatory compliance.

Qualifications

• Associate's or Bachelor's Degree in Nursing (Required).
• Minimum 1–2 years of clinical nursing experience (Preferred).
• CCM certification preferred.

Responsibilities

• Assesses patient needs and develops individualized care management plans.
• Coordinates patient care, referrals, and healthcare services across departments.
• Collaborates with physicians, nursing staff, and multidisciplinary teams to support continuity of care.
• Monitors patient progress and identifies barriers to treatment or recovery.
• Facilitates discharge planning and coordinates post-discharge services.
• Educates patients and families about treatment options, available resources, and care plans.
• Supports appropriate utilization of healthcare resources and adherence to clinical guidelines.
• Maintains accurate care management documentation and follows regulatory and patient safety requirements.
