Workforce Recruiter is seeking a Patient Care Manager (RN) to lead our home health team in California. You will supervise clinical staff, ensure care aligns with standards, and partner with families and referral sources to deliver compassionate services.

This role drives quality improvement, supports staff development, ensures regulatory compliance, and promotes safe, effective patient care across the organization.

Competenze

• RN with current license in the practicing state.
• 5 years RN experience with home health setting.
• 2 years management or supervisory experience in home health (preferred).
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

• Provide clinical supervision to ensure care aligns with standards, policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care for high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and staff development.
