COPE Health Solutions invites a Care Manager RN to join our team, supporting the Care at Home Solutions program from a remote Los Angeles, CA setting. The role centers on conducting assessments, building individualized care plans, and collaborating with physicians, pharmacists, and social workers to improve outcomes.

The position emphasizes care transitions, behavioral health screening, and coordinating resources to reduce unnecessary utilization while maintaining high-quality patient care.

Qualifikationen

• RN License – California Licensure preferred in addition
• Bachelor’s degree in nursing preferred; Associate degree in nursing is minimum requirement
• 1-2 years’ experience in acute inpatient, rehabilitation, sub-acute, skilled facility, home care, ambulatory care management, or managed health plan
• Preferred: CCM certification
• Preferred: Care/Case Management experience

Aufgaben

• Evaluates patients for care management services and determines level of care coordination.
• Completes comprehensive assessments to identify risk and develop care plans.
• Collaborates with physicians, pharmacists, dieticians, social workers and others to create person-centered plans.
• Monitors and updates care plans with progress toward SMART goals.
• Coordinates referrals and authorizations within care management areas.
