Job summary

Mercy Health is hiring an RN Care Manager at Springfield Regional Medical Center to coordinate patient care, plan transitions, and collaborate with a multidisciplinary team. You will assess needs, document plans, and promote evidence-based discharge planning while ensuring patient and family engagement in decisions.

The role requires bedside nursing expertise and strong communication. We seek candidates with a BSN and an active RN license.

Qualifications

• BSN preferred; required for RSFH.
• RN licensure in the working state or compact.
• ACM/ CCM/ ANCC certifications preferred.

Responsibilities

• Identify and prioritize patients needing care management.
• Document care plans clearly and timely in patient records.
• Coordinate transitions across care settings and teams.
• Promote adherence to standardized advance care planning.
• Support discharge planning and resource utilization.
• Assist in removing barriers to timely testing and treatment.
