Job summary

Endeavor Health is seeking an RN Care Manager in Evanston, IL for a full-time, 40-hour week with rotating weekends and holiday coverage. The role includes managing payor authorizations, coordinating across the care continuum, and ensuring documentation reflects severity of illness and service intensity.

Requirements include IL RN licensure, a BSN (NLN-accredited preferred), and 2+ years of clinical nursing experience with case-management skills.

Qualifications

• State nursing licensure in Illinois is required.
• BSN with NLN-accredited nursing school preferred.
• Clinical certification in case management is beneficial.
• BL/CPR certification required or as listed by employer.
• Minimum 2 years of clinical nursing experience required; care management experience preferred.
• EMR experience and knowledge of care-criteria systems preferred.

Responsibilities

• Facilitate payor authorization processes and ensure correct patient type.
• Collaborate on patient care across the continuum to remove barriers to care and reimbursement.
• Coordinate with CDI team to reflect severity of illness and service intensity.
• Maintain communication with physicians, nursing, and care team for timely progression.
• Resolve system problems affecting diagnosis or treatment progress.
• Identify and resolve discharge delays to optimize length of stay.
• Seek consultations to expedite care and discharge when needed.
• Monitor patient progress and ensure high-quality, cost-efficient services.
