Job summary

Qualifications

• Active IL nursing license.
• Bachelor's degree in Nursing; NLN-accredited program preferred.
• Clinical certification beneficial; BLS/CPR required.
• Minimum 2 years nursing experience; UM or chronic disease management preferred.

Responsibilities

• Facilitates payor authorization processes and ensures accuracy of patient type.
• Manages patient care across the continuum to remove barriers to timely care and reimbursement.
• Ensures documentation reflects severity of illness and intensity of service with the CDI team.
• Collaborates with physicians, nursing, and multi-disciplinary team for timely patient progression.
• Addresses obstacles to discharge and expedites care and discharge when needed.
• Seeks consultations to expedite care and maintain efficient discharge.
• Monitors progress to ensure a high-quality, cost-effective care plan.
