Job summary

Texas Health Resources in the Dallas–Fort Worth area is seeking a Registered Nurse Outpatient Care Manager for PRN remote placement. The role requires coordination of care for members with disease management programs and collaboration with physicians and the care team.

You will conduct assessments via phone or in person, develop patient-centered care plans, and ensure compliance with policies and regulatory standards while maintaining a positive, fast-paced work environment.

Qualifications

• Active RN license at hire.
• Associates Degree in Nursing required; BSN preferred.
• 3 years managed care or case management experience required.
• Utilization Management and pre-authorization experience preferred.

Responsibilities

• Coordinate care, education, and disease management for enrolled members.
• Support Outpatient Care Management by addressing needs with the care team.
• Communicate effectively with internal and external clients.
• Manage caseload with patient assessment via phone or in person.
• Ensure tasks align with company strategy, goals, and timelines.
