North Carolina Community Health Center Association seeks an RN Care Manager to bridge central clinical teams with practice-based centers. You will prioritize patient needs, coordinate interventions, and monitor progress toward quality and cost-effective outcomes, with emphasis on transitions of care across the continuum.

The role requires RN licensure in NC, at least three years in care management, and strong EMR and communication skills.

Formación

• Associate degree or better from an accredited School of Nursing.
• Licensed as a Registered Nurse (RN) with the state of NC.
• At least three years of experience providing care management, case management, or care coordination to a safety-net population.
• CCM certification preferred.

Responsabilidades

• Oversee care management services for enrolled populations per standards of practice.
• Develop comprehensive patient-centered assessments across medical, biopsychosocial, behavioral, spiritual, and cultural needs.
• Identify barriers and supports with patients and caregivers to improve treatment access and outcomes.
• Plan and coordinate transitional care and discharge instructions with the care team.
• Provide patient education on diagnoses, medications, resources, prevention, and self-management.
