Job summary

Humana is seeking a RN Care Manager in Wisconsin with no night or weekend shifts to support the Wisconsin Family Care Program. You will coordinate care, educate members, and maintain documentation while traveling up to 40% of the time.

The role is remote from home with regular hours and the opportunity to impact member well-being. Responsibilities include assessing members, developing MCPs, coordinating with providers, and supporting new staff through documentation and care transitions.

Qualifications

• Associate degree in nursing is required.
• Licensed Registered Nurse in the state of Wisconsin in good standing is required.

Responsibilities

• Assess members and develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise and lifestyle modifications.
• Document in member files within required DHS contract timeframes.
• Travel to member visits and perform in-person visits as needed.
• Ensure timely remediation and follow-up in D365 system and handoffs to new RNs.
