LCMC Health seeks a dedicated Care Manager to coordinate patient care, develop individualized plans, and educate on self-management. You will liaise with care teams, discharge planners, and providers to ensure optimal outcomes and appropriate resource use.

Your nursing background informs decisions and advocacy across the continuum of care. You will perform outbound assessments, manage referrals, and document workflows in EPIC, while guiding patients and families through complex care journeys

Qualifications

• Current licensed registered nurse in Louisiana with active practice.
• Minimum 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans with prioritized goals considering patient and caregiver involvement.
• Make outbound calls to assess health status and identify barriers in treatment plans.
• Coordinate care and referrals to external sources; ensure proper discharge services and medication reconciliation.
• Triage symptom-based calls per protocols and enroll patients in home monitoring programs.
• Educate patients and families on self-management and chronic/acute disease processes.
• Document all workflows in EPIC for tracking and quality assessment; collaborate with care partners to meet quality and cost goals.
