LCMC Health in Louisiana seeks a Care Manager to be the hub of patient case management and care planning. You will coordinate care across the care team, develop individualized plans, and support patients and families through transitions.

You’ll use nursing expertise and compassionate problem-solving to optimize outcomes, lengths of stay, and resource use while educating patients for ongoing self-management.

Qualifikationen

• Current nursing license to practice in Louisiana.
• 2 years of professional nursing or care management experience.

Aufgaben

• Develop individualized care plans with goals aligned to patient and caregiver preferences.
• Make outbound calls to assess patient health status and coordinate care.
• Identify gaps or barriers in treatment plans and arrange referrals as needed.
• Coordinate care during hospital discharge or ED transitions and ensure medication reconciliation.
• Document workflows in EPIC and educate patients for self-management of wellness and disease.
