Job summary

LCMC Health in Louisiana is seeking a Care Manager to be the hub of patient case management and care planning, coordinating with the care team to achieve quality outcomes and efficient resource use. The role requires an active Louisiana nursing license and at least two years of professional nursing or care management experience.

You will develop individualized care plans, make outbound calls, coordinate referrals, ensure safe discharge, educate patients, and document workflows in EPIC.

Qualifications

• Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
• 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans considering patient and caregiver goals.
• Make outbound calls to assess patient health status.
• Coordinate care and referrals to outside sources.
• Ensure discharging patients receive necessary services and resources.
• Triage symptom-based calls per approved protocols.
• Enroll patients and manage alerts for home monitoring programs.
• Educate enrolled members on self-management of wellness and disease.
• Document workflows in EPIC for tracking and quality assessment.
• Collaborate with care team to discuss clinical questions and care plans.
