Cypress Medical Management Group in Monterey, CA seeks an RN Care Manager to lead care coordination for patients with complex needs. The role partners with PCPs, specialists and care teams to develop comprehensive care plans, monitor outcomes, and promote evidence-based practices.

The position emphasizes collaboration, patient advocacy, and community resource navigation, with a focus on high-quality patient care and satisfaction.

Competenze

• Current RN licensed in California.
• American Heart Association BLS.
• Minimum one year recent clinical nursing experience.

Mansioni

• Assesses the healthcare, educational, and psycho social needs of patients and their families.
• Collaborates with patient/family, PCP and care team to develop short and long term health goals and a detailed Care Plan.
• Partners with PCPs to ensure care adherence and integrate guidelines and protocols; initiates or adjusts therapies and provides follow up.
• Triages incoming referrals; assists with scheduling appointments; conducts intake visits; coordinates care and pre-visit planning.
• Develops and implements self-management plan with patient to prevent exacerbation and intervene during disease progression.
• Educates and documents patient care plan goals and measures for better health control.
• Makes referrals to community resources and coordinates communication with family.
