Job summary

Community Health Choice, Inc. in Houston, TX seeks a Care Manager to oversee members with chronic disease and complex needs. You will coordinate care, assess and stratify risk, and partner with providers to implement care plans.

This role supports utilization review, member services and quality improvement teams. The ideal candidate has an RN license in Texas, CCM credential (or willingness to obtain within 18 months), 3 years clinical experience, and 2+ years in an MCO setting.

Qualifications

• Associate's Degree required.
• RN license (Texas) required; CCM certification required or must be pursued within 18 months.
• Three (3) years of clinical experience.
• Two (2) years or more of MCO experience.
• Microsoft Office proficiency (Word, Excel, Outlook).

Responsibilities

• Caseload management and timely documentation in care management system.
• Initial assessment and risk stratification of identified members.
• Coordinate with providers and care teams; develop and implement care plans.
• Perform ongoing interventions; provide member education on self-management.
• Collaborate on quality/process improvement and program development.
• Maintain knowledge of program requirements and disease processes.
• Support achievement of departmental goals per annual plan.
