Job summary

Qualifications

• Current RN licensed in California.
• American Heart Association BLS.
• Minimum one year of clinical nursing experience.
• Experience in care management or Medicare/Medicaid environment preferred.

Responsibilities

• Assess patients' healthcare, educational, and psychosocial needs.
• Develop care plans and coordinate with PCPs and care teams.
• Triages patients and coordinates referrals and follow-ups.
• Ensure adherence to evidence-based guidelines and transitions of care.
• Collaborate with hospital staff during ER/ward events.
• Maintain metrics to improve care delivery and outcomes.
