Job summary

McLaren Health Care seeks an RN Care Manager to assess, plan, implement, coordinate, monitor, and evaluate health services for chronically ill patients within a care management framework.

You will work with an interdisciplinary team to develop individualized care plans, coordinate with PCP and specialists, ensure continuity of care, and promote patient self-management and outcomes improvement.

Qualifications

• RN license in Michigan is required.
• Three (3) years of clinical nursing experience serving chronically ill patients.

Responsibilities

• Provides telephonic and face-to-face comprehensive assessment and care management services.
• Uses multi-dimensional assessment to target high-risk populations.
• Coordinates care with PCP, hospitals, and community resources.
• Documents care management activities and monitors patient response to the plan of care.
• Promotes patient self-management and caregiver engagement.
• Reviews current literature to improve engagement and care management practices.
