Job summary

LCMC Health in Louisiana is seeking a Care Manager responsible for coordinating patient care across the continuum. You will develop individualized care plans, perform outbound health assessments, and coordinate discharges with medication reconciliation.

The role emphasizes collaboration with care teams to improve outcomes and cost management. The position requires an active Louisiana nursing license and at least two years in nursing or care management, with hands-on use of EPIC for workflow

Qualifications

• Current nursing license to practice in Louisiana as defined by the Louisiana State Board of Nursing.
• 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans considering patient and caregiver goals.
• Make outbound calls to assess patients’ current health status.
• Identify gaps or barriers in treatment plans.
• Coordinate care and make referrals to outside resources.
• Ensure discharge services and med reconciliation are arranged.
• Triage symptom-based calls per approved protocols.
• Enroll patients and manage alerts for home monitoring programs.
• Provide approved general health information to callers.
• Document workflows in the EPIC system for tracking and quality.
• Educate patients to assist with self-management of wellness and disease.
• Educate on chronic and acute disease processes.
• Collaborate with care partners to discuss plans and outcomes.
