Job summary

LCMC Health in Louisiana is seeking a Care Manager to serve as the hub of patient case management and care planning. You will develop individualized care plans, coordinate with the care team, and ensure smooth transitions from hospital to home while focusing on quality, appropriate length of stay, and cost management.

You will make outbound calls to assess health status, document workflows in EPIC, educate patients for self-management, and collaborate with physicians and care partners.

Responsibilities

• Develop individualized care plans, including prioritized goals, that consider patient and caregiver preferences and involvement in the plan.
• Make outbound calls to assess patient health status.
• Identify gaps or barriers in treatment plans.
• Coordinate care and make referrals to outside sources.
• Ensure patients discharged from hospital receive necessary services and resources, including medication reconciliation.
• Triage symptom-based calls per approved protocols for a range of patients.
• Enroll patients and manage alerts for home monitoring programs.
• Provide general health information and guidance to callers from approved sources.
• Document all workflows in EPIC for tracking and quality assessment.
• Educate enrolled members on self-management of wellness and disease.
• Educate on chronic and acute disease processes.
