Boston Medical Center (BMC) seeks a Registered Nurse Care Manager to coordinate and optimize patient care across the continuum. The role focuses on assessing, planning, implementing, monitoring, and evaluating services to meet health needs while promoting quality outcomes and cost efficiency.

The position supports the care management process and department, handling increased volume and collaborating with physicians, payers, and families to ensure timely, value-driven care.

Qualifications

• Graduate of an accredited BS program in Nursing preferred.
• Licensed to practice professional nursing in Massachusetts.
• 3–5 years of clinical experience required.
• 1–3 years in acute, inpatient care management or TCM program completion.

Responsibilities

• Assess, plan, implement, monitor and evaluate care options.
• Identify providers and facilities across the continuum.
• Promote cost-effective, timely care and resource use.
• Support the care management process and department.
