Job summary

Boston Medical Center (BMC) in Boston, MA is seeking an RN Care Manager Float for our Case Management department. This role supports a designated patient caseload, coordinates care across the continuum, and promotes efficient, cost-effective utilization of resources.

You will collaborate with physicians, nurses, and other professionals; participate in discharge planning, utilization management, and quality initiatives; requires a nursing degree and active MA license.

Qualifications

• A minimum of 1-3 years of recent acute inpatient care management experience.
• CCM or related certification preferred.
• Licensed to practice professional nursing as a Registered Nurse in Massachusetts by start date.

Responsibilities

• Assess a client’s status and develop an individualized care management plan.
• Coordinate care across the continuum and manage discharge planning.
• Collaborate with physician, nursing, and multidisciplinary care team to ensure timely and appropriate patient management.
• Utilize utilization management and quality screening for assigned patients; monitor LOS and resource use.
• Document all plan of care and patient progress in the medical record per department standards.
