Boston Medical Center (BMC) is seeking an RN Care Manager to oversee a designated patient caseload within the 3 Surgical Step Down unit. The role emphasizes assessment, planning, implementation, and coordination of care across the continuum to achieve quality outcomes and cost-effective utilization of resources.

The position requires an active Massachusetts RN license, a BSN, and 1–3 years of acute care management experience, with CCM preferred.

Qualifications

• Nursing degree from an accredited program.
• Licensed to practice as a Registered Nurse in Massachusetts by start date.
• CCM or related certification preferred.
• 1–3 years of acute inpatient care management experience required.
• 2 years in judgement/decision making, preferably in utilization management.

Responsibilities

• Evaluate client status across physical, psychosocial, and environmental factors.
• Coordinate care progression across continuum of care.
• Collaborate with physicians, nurses, and multidisciplinary team.
• Monitor length of stay and resource use; participate in discharge planning.
• Communicate with third-party payers and document in records.
• Utilize acuity criteria to monitor admissions and stays.
