Job summary

Molina Healthcare, Inc. is seeking a care management RN to support LTSS activities across the care continuum. You will collaborate with a multidisciplinary team to coordinate member care, assess needs, and implement effective care plans to improve outcomes while controlling costs.

The role covers multiple counties and includes in-person home visits, waiver processes, and ongoing monitoring. Travel up to 40% may be required as you engage with providers, members, and families to navigate complex

Qualifications

• At least 2 years of experience in health care, including 1 year in care management or LTSS.
• RN license active in state of practice.
• Bachelor's degree may be required in certain states.
• Valid driver's license and reliable transportation for travel.
• Able to work independently, with strong communication and time-management skills.
• Experience with substance use disorders or disability populations is a plus.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, and providers.
• Monitor care plans and evaluate outcomes, adjusting as needed.
• Promote integrated services including behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service decisions and documentation.
• Identify barriers and support members in psycho/social and medical areas.
• Travel 25-40% as required by state/contractual requirements.
