Molina Healthcare, Inc. is seeking an RN-led Care Manager to coordinate long-term services and supports (LTSS) for high-need members. You will work with a multidisciplinary team to assess needs, develop comprehensive care plans, and ensure timely service delivery while monitoring outcomes and addressing barriers to care.

The role requires strong communication, clinical knowledge, and the ability to work independently across diverse settings. Travel up to 40% locally is expected.

Formación

• Active RN license in state of practice.
• At least 2 years in health care with care management or LTSS exposure.
• Proficient with standard office software.
• Willingness to travel 25–40% locally as required.

Responsabilidades

• Complete comprehensive member assessments within timelines, including home visits as required.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plan effectiveness and adjust as needed.
• Promote service integration across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Support cross-team collaboration and ICT meetings.
• Provide patient education and motivational support.
• Identify barriers to care and address psycho/social and medical obstacles.
• May conduct medication reconciliation for complex cases.
