Job summary

Molina Healthcare is seeking a Registered Nurse care manager in Wisconsin to support LTSS care coordination, collaborate with a multidisciplinary team, and drive high-need members toward outcomes across the continuum.

You will conduct comprehensive assessments, develop and monitor care plans, coordinate waiver services, and facilitate ICT meetings while addressing barriers to care and coordinating with providers and caregivers. Travel up to 25-40% may be required.

Qualifications

• Must hold an active RN license in state of practice.
• Experience in care management, managed care, or LTSS preferred.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Develops and implements care plans, including waiver service plans with members and care networks.
• Monitors care plans, documents interventions, and suggests changes to improve outcomes.
