Job summary

Molina Healthcare seeks a Registered Nurse care manager to coordinate long-term services and supports (LTSS) and care across the continuum for high-need members. You will collaborate with multidisciplinary teams and ensure progress toward outcomes while supporting strategy for quality and cost-effective member care.

Responsibilities include comprehensive assessments, waiver enrollment/disenrollment, care plan development, and monitoring. Travel up to 40% may be required based on state contracts.

Qualifications

• Active RN license in state of practice.
• Minimum 2 years healthcare experience incl care management.
• Valid driver's license and reliable transportation.
• Ability to work independently and with diverse populations.

Responsibilities

• Perform comprehensive member assessments within timelines, including in-person home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members and care team.
• Promote integration of LTSS and behavioral health services.
• Use motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Travel 25-40% local as required.
