Job summary

Molina Healthcare is seeking a Nurse Care Manager to support care management and LTSS activities. You will collaborate with multidisciplinary teams to coordinate member care across the continuum for high-need individuals, ensuring progress toward outcomes and alignment with strategy.

The role requires an active RN license, 2+ years in health care with care management experience, and travel as needed. Competitive benefits are included.

Qualifications

Registered Nurse (RN) license active and unrestricted.
At least 2 years in health care with 1 year in care management or LTSS.
Experience with persons with disabilities or chronic conditions is preferred.
Must have valid driver’s license and reliable transportation for travel requirements.
Strong communication and time-management skills.
Ability to work independently with minimal supervision.

Responsibilities

Complete comprehensive member assessments within timelines, including home visits as required.
Facilitate waiver enrollment/disenrollment processes.
Develop and implement care plans with members and care teams.
Monitor care plans and document interventions and outcomes.
Promote integration of LTSS and behavioral health services.
Assess medical necessity and authorize waiver services.
