Job summary

Molina Healthcare is seeking a Registered Nurse with care management expertise to coordinate LTSS across the continuum. The role involves assessments, care planning, and collaboration with an interdisciplinary team to improve member outcomes and control costs.

Travel to member homes and community settings will be required as per state contracts; strong communication and organizational skills are essential for success in a fast-paced, regulated environment.

Qualifications

• RN license active and unrestricted in state of practice.
• 2+ years total healthcare experience incl. care management/managed care.
• Experience with persons with disabilities, chronic conditions, substance use disorders, and LTSS is preferred.
• Valid driver's license and reliable transportation for travel requirements.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and others.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of services across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
