Job summary

Molina Healthcare is seeking an experienced RN Care Manager to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and ensure progress toward outcomes.

The role involves waiver services, care plan development, and proactive problem-solving to address barriers to care, with 25–40% local travel as required by state contracts.

Qualifications

• At least 2 years in health care with care management experience.
• RN license active and unrestricted in state of practice.
• Bachelor’s degree may be required per state/contract.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Develop and implement care plans with members, caregivers, and providers.
• Monitor care plans and adjust interventions to achieve outcomes.
• Facilitate waiver enrollment/disenrollment and coordinate LTSS services.
• Promote integration of behavioral health and LTSS with community resources.
• Collaborate with interdisciplinary care teams for approvals/denials.
