Qualifications

• RN license active and unrestricted in state of practice.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of services across care teams and LTSS.
• Assess medical necessity and authorize waiver services.
• Communicate with multidisciplinary teams and document interventions.
• Travel 25-40% as required by state/contract.
• Educate and motivate members using motivational interviewing.
