Qualifications

• Active RN license in state of practice.
• 2+ years in health care including care coordination or managed care.
• Experience with persons with disabilities or LTSS preferred.
• Valid driver's license and travel ability for regional work.

Responsibilities

• Perform comprehensive member assessments within timelines and conduct home visits as needed.
• Coordinate waiver enrollment/disenrollment and develop care plans with the team.
• Monitor care plans, document outcomes, and adjust as required.
• Promote integrated care and connect members with community resources.
• Participate in ICT meetings and provide guidance on care decisions.
• Conduct medication reconciliation as needed and manage complex cases.
