Qualifications

• Minimum 2 years of health care experience including care management or managed care.
• Active RN license in state of practice.
• Bachelor's degree may be required in some states.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently and in diverse settings.
• Strong verbal and written communication; proficient in case management.

Responsibilities

• Complete comprehensive member assessments within timelines.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members and care team.
• Monitor interventions and goal achievement; document accordingly.
• Promote LTSS integration and connect to community resources.
• Assess medical necessity and authorize waiver services.
• Lead ICT meetings for service decisions.
• Perform medication reconciliation when needed.
• Travel 25-40% as required by state/contractual requirements.
