Job summary

Molina Healthcare is seeking an RN Care Manager to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and contribute to high-quality, cost-effective outcomes.

The role requires 2+ years in health care with care management or LTSS experience, an active RN license, and a valid driver’s license. Travel 25–40% may be required based on state contracts.

Qualifications

• RN license active and unrestricted in state of practice
• 2+ years of health care experience with care management or LTSS
• Driver's license and reliable transportation for travel up to 40%

Responsibilities

• Complete comprehensive member assessments within regulatory timelines and sometimes in-home visits
• Facilitate waiver enrollment/disenrollment and implement care plans with care teams
• Monitor care plans, document interventions, and adjust as needed
• Coordinate LTSS, behavioral health, and community resources for seamless care
• Support ICT meetings for service approvals and collaborate with team members
• Identify barriers to care and address psycho/social, financial, and medical obstacles
• Possibly conduct medication reconciliation and manage complex cases when assigned
