Job summary

Responsibilities include care plan monitoring, coordinating with ICT meetings, and educating members to overcome barriers. Travel to member homes is required 25-40% of the time depending on state requirements.

Qualifications

• At least 2 years in healthcare with 1 year in care management or managed care.
• RN license active and unrestricted in state of practice.
• Valid driver's license and reliable transportation for travel.
• Strong communication, organization, and ability to work independently.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans, including a waiver service plan with members, caregivers, physicians and care network.
• Monitor care plans, document interventions and goal achievement, and adjust as needed.
• Promote integration of services for LTSS and behavioral health to ensure continuity of care.
• Assess medical necessity and authorize waiver services as appropriate.
• Support and educate members using motivational interviewing and Molina clinical guideposts.
• Travel 25-40% locally as required.
