Job summary

Molina Healthcare is seeking a skilled Registered Nurse Care Manager to support long-term services and supports (LTSS) care coordination across the care continuum. You will work with a multidisciplinary team to ensure integrated, quality, cost-effective member care.

Responsibilities include member assessments, care plan development, waiver service management, and ongoing plan monitoring. Travel up to 25-40% may be required to fulfill duties and engage with members in various settings.

Qualifications

• 2 years of healthcare experience including at least 1 year in care management or similar setting
• RN license active and unrestricted in state of practice
• Bachelor's degree in a healthcare-related field may be required in some states
• Valid driver's license and reliable transportation for travel
• Ability to work independently with minimal supervision and handle multiple priorities
• Strong communication skills, both written and verbal
• Knowledge of community resources and LTSS programs
• Experience with substance use disorders in some states is a plus
• Experience collaborating with multidisciplinary teams and coordinating care across settings

Responsibilities

• Complete comprehensive member assessments within required timelines, including in-person home visits
• Facilitate waiver enrollment/disenrollment processes
• Develop and implement care plans with members, caregivers, and providers
• Monitor care plan effectiveness and adjust as needed
• Coordinate LTSS and behavioral health services and resources
• Assess medical necessity and authorize waiver services
• Lead ICT meetings for service approvals or denials
• Travel to member locations as required (25-40%)
